Advance Care Planning Appointments

Honoring Choices Virginia offers free advance care planning appointments with a trained facilitator every Friday from 12:00 to 4:30pm. Start the process, learn how to talk with loved ones, and make sure that your wishes will be honored. Appointments are open to all Virginians, healthy or living with well managed chronic illness.

Advance Care Planning 101

What if a sudden illness or injury left you unable to speak for yourself? Who would you want to speak for you? What would you want them to know about your values and wishes? Advance Care Planning (ACP) is a conversation process about the medical care you would want if you could not speak for yourself, and needed others to make decisions for you. Advance care planning helps you think about health care choices you may have to make in the future; talk about your choices with your doctor and your loved ones; and make a written plan for the future (advance directive). Learn about the resources available to assist individuals in developing a thoughtful, person-centered advance care plan.
